Dear Sir,

With reference to above, kindly attach here with the form required to fill by us and signed one for our claim and note the following.

1. Our employ who injured severely have been treated partially by Rashid Hospital. Now the hospital    instructing to shift him for the further treatment to other hospital. As the body below the waist not moving and other problems.
2. The claim of the Rashid Hospital will be submitted once we received their claim after discharge the patient from the hospital.
3. Kindly advise the name of the hospital for the future treatment in Dubai’ since the future claim by the hospital will be reimburse by your company under our project insurance as instructed by Dubai police.
4. The police case hold the passport will not be closed until the patient will not be recover here Dubai.so kindly advise the potential hospital & attaching the after visit summary.

So, find the attached document and we are waiting your immediate action because Rashid Hospital management giving the warning to discharge the patient  from the bed if we will not provide the other hospital detail were we need to shift  for the full recovery.

 Kindly advise imidiation we will submit the care paper form the police by Monday & Tuesday.
