With reference to the above, and as per your letter ref no:AP/269/CC/243 DT.15.02.17, We have repeatedly intimated the pool subcontractor to attach the covering paper along with the DM form with their statement because we received the DM form as a loose paper, without stating the purpose of the document and we called the pool subcontractor to attach the Environmental document along with the covering sheet
So, till date the pool subcontractor have not attached along with the covering sheet , So, we are submitting the received 03 sheets of documents to your office , please review and let us know it will fulfill your requirement or not.
